Bug Tracker Needed

My faithful pen-and-paper bug log is becoming less and less effective each time I take on a new development project.
I am looking for suggestions / recommendations for a good bug-tracker application.
Basic prequisites for the system are that it must be web-based, free (as in cost), open-source, compatible with MySQL and written in either Perl or PHP. It must be capable of tracking several concurrent projects and it must support multiple contributors and developers.
I am currently considering Mantis and Bugzilla, with a bias towards Mantis.
How do you keep track of bugs? What software do you use? Let me know what other packages I should be checking out.
